About Us Maple Valley Nursing Home is a Skilled Nursing Facility licensed by both Michigan Medicaid and the Federal Medicare Program. Maple Valley Nursing Home is privately owned by John & Donna Kasben. John has been the Administrator of Maple Valley since 1996. His wife Donna, is the Director of Nursing and has over 25 years of experience at Maple Valley. Our medical director is Dr. Mark Barber, DO. He specializes in Family Practice. Naturally, we cannot list the names of all of our staff, but they are all very important. We take pride in the fact that we have one of the lowest turnover rates of any Nursing Facilities in the area.

Rates We accept the following types of Payments for Services at Maple Valley Nursing Home: Medicaid, Medicare, Private Pay and Other Insurances. Medicaid - The Michigan Medicaid Program will pay for Room & Board Services if you are eligible for Medicaid Payment. The Resident will be responsible for the monthly payment of the Patient Pay Amount that is determined by the State of Michigan. Medicare - To be eligible for Medicare Part A services, the following 3 criteria must be met: (1) Must have Medicare Part A Coverage in effect. (2) Resident must have a 3-day Qualifying Hospital Stay. (3) The Resident must have Skilled Nursing Needs (Therapy, IV's, Oxygen etc.). Medicare Part B - Medicare Part B Coverage does not include Room & Board Charges. However, Medicare may cover certain services such as Part B Therapies. Private Pay - Residents who are not eligible under the Medicare Part A or the Michigan Medicaid Program will be charged our Private Pay Rate for their Room & Board and certain incidentals (listed under the Other Charges Section). Our current Private Pay Rate is $180.00 per day. The monthly room & board bill is due on the 5th of the month. If payment is received by the 1st of the month, we offer a discount of $3.00 per day. Other Insurances - We will accept payments from other Insurance Carriers, but arrangements must be made prior to admission to Maple Valley Nursing Home for Room & Board Charges. Maple Valley will bill other Insurance Companies for charges such as Medicare Part A and B Co-insurance and Deductibles. *******OTHER CHARGES******* Cable Television is available for a monthly charge of $20.00. Residents must bring in their own Television. Incontinence Products are available for Private Residents for a monthly charge of $45.00. Haircuts are provided on the first Wednesday of every month. The charge for a haircut is $15.00 for women and $10.00 for men. Perms are also available at Maple Valley Nursing Home for a charge of $50.00. All haircuts and perms are done in our "Beautician's Lounge" also known as the "Training Room".
