Maple Valley Nursing Home understands that the decision to place a loved one is a hard, heart-wrenching decision.  Our main goal is to make your loved one part of our family and to make the transition as easy as possible.

Our Licensed Master of Social Work Professional will offer One-on-One counseling with Residents and their family when needed to help in this difficult time.

Family Support is also offered on a regular basis.

Our Social Worker has many responsibilities including:

• Discharge Planning – We will help set up all the necessary services that will be needed when a Resident can be discharged safely back to theirs or a loved one’s home.
• Psychiatric and Hospice Referrals as well as Palliative Care Referrals.
• Mood and Behavior tracking with care plan implementation and
• Monitoring of Psychoactive medications.

.Other Responsibilities include providing periodic inservices to staff regarding dementia care, depression and anxiety treatment and other pertinent topics.
